Job Summary:

We are seeking a compassionate and dedicated Support Worker / Health Care Assistant to join our Neurological Rehabilitation team. The ideal candidate will provide essential support and care to individuals recovering from neurological injuries or living with chronic neurological conditions. This role requires a person-centered approach to assist patients in their rehabilitation journey, promoting independence and improving their quality of life.

Key Responsibilities:

  • Assist patients with daily living activities, including personal care, meal preparation, and mobility.
  • Support the implementation of individual care plans designed by healthcare professionals.
  • Monitor and report changes in patients’ conditions and well-being.
  • Facilitate and encourage patient participation in therapeutic activities and exercises.
  • Provide emotional support and companionship to patients and their families.
  • Maintain accurate records of care provided and progress made.
  • Ensure a safe and clean environment for patients.
  • Collaborate with the multidisciplinary team to deliver high-quality care.
  • Attend training sessions and team meetings as required.

Qualifications:

  • NVQ Level 2 or 3 in Health and Social Care, or equivalent is preferred.
  • Previous experience in a care setting, particularly in neuro rehab or similar, is highly desirable.
  • Understanding of the specific needs of patients with neurological conditions.
  • Excellent communication and interpersonal skills.
  • A compassionate, patient, and caring nature.
  • Ability to work flexibly across various shifts, including nights and weekends.
  • Commitment to the values of dignity, respect, and empathy.

Skills:

  • Good observational skills to assess patient conditions and recognize any changes.
  • Strong organizational skills and the ability to prioritize tasks.
  • Ability to work both independently and as part of a team.
  • Physical fitness to assist with patient mobility and therapeutic activities.
