Zirojob, in partnership with Liverpool City Council, is seeking a dedicated Adult Social Worker to join our busy hospital discharge team. The successful candidate will work across multiple hospital sites within the Liverpool area, ensuring the provision of exceptional care and support to adults during the discharge planning process.
Key Responsibilities
- Conduct comprehensive assessments of patients’ needs for post-hospital services.
- Work collaboratively with healthcare professionals to create effective discharge plans.
- Facilitate safe and timely discharges from hospital to reduce the risk of re-admission.
- Provide supportive counseling and advocacy to patients and their families.
- Maintain accurate records and documentation in compliance with legal and ethical guidelines.
- Offer guidance on community resources and make appropriate referrals where necessary.
Must-Haves:
- HCPC (Health and Care Professions Council) Registration.
- Enhanced DBS (Disclosure and Barring Service) check.
- A minimum of 2 years of experience in social work, preferably within a hospital or health care environment.
Qualifications
- Bachelor’s degree in Social Work (BSW) or Master’s degree in Social Work (MSW).
- Strong knowledge of local resources and systems relevant to adult social care.
- Excellent communication and interpersonal skills, with an ability to work effectively in a multidisciplinary team.
- Demonstrated ability to handle complex situations with a patient-centered approach.
