Job Summary:

We are seeking a compassionate and experienced Social Worker to join our Adult Social Care team. The successful candidate will be responsible for carrying out comprehensive social care assessments in hospitals, including mental health and general hospitals, with the primary goal of facilitating timely and safe discharges into the community.

Key Responsibilities:

  • Conduct thorough assessments of adults over the age of 67 to determine their social care needs.
  • Develop and implement personalized care plans to support safe discharge into the community.
  • Collaborate with multidisciplinary teams, including healthcare professionals and care providers, to coordinate discharge plans.
  • Provide support and guidance to patients and their families throughout the discharge process.
  • Advocate for patients’ rights and ensure that their voices are heard in decision-making processes.
  • Maintain accurate and up-to-date records of assessments, care plans, and outcomes.
  • Participate in ongoing professional development to stay informed about best practices in adult social care.

Must-Have:

  • A degree in Social Work and registration with the relevant social work body.
  • A minimum of 2 years’ experience in adult social care, preferably within a hospital discharge setting.
  • Excellent knowledge of legislation and policies related to adult social care.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
  • Proven ability to manage complex cases and make sound decisions under pressure.
  • A valid driver’s license and access to a vehicle, as travel to various hospital sites is required.
  • Commitment to person-centered care and a passion for making a positive impact on the lives of adults.
